Dog Adoption Application

Arizona Poodle Rescue adult dogs are spayed or neutered (unless medical condition or age related delay) and are current on vaccinations. The adopter agrees to have the dog vet checked by the adopter’s personal veterinarian within a 14 day period. Should medical issues arise the dog may be returned to APR and the adoption fee will be reimbursed. APR will not pay for any vet fees after the adoption contract is executed unless otherwise agreed by APR representative. There is no guarantee or warranty of any kind in connection with any medical and/or dental conditions and/or expenses; past, present or future. I understand that this is a preliminary application and that there may be no Dog available in rescue at this time. I understand that if a Dog should become available, that I will be required to provide further information including, but not limited to, personal and veterinary references and availability of a home check. I further understand that all information on my Preliminary Rescue Adoption Application will be kept confidential; that my application will be kept on file until I notify Arizona Poodle Rescue that I am no longer interested in a rescue; and that I will not be contacted unless an appropriate Dog becomes available.
