I understand that this is a preliminary application and that there may be no dog available in rescue at this time. I understand that if a dog should become available, that I may be required to provide further information including, but not limited to, personal and veterinary references and a home visit. I further understand that all information on my Preliminary Rescue Foster Application will be kept confidential; that my application will be kept on file until I notify the Arizona Poodle Rescue that I am no longer interested in a rescue; and that I will not be contacted unless an appropriate dog becomes available. As a volunteer of this rescue, I agree to abide by the policies and procedures. I understand that I will be volunteering at my own risk and that the rescue, its Board of Directors, other volunteers, employees and affiliates, cannot assume any responsibility for any liability for any accident, injury to human or animal(s), health problem(s) or damage which may arise from any volunteer work I perform for the rescue.
