Dog Adoption Contract

I am adopting the dog described above through Arizona Poodle Rescue. I will be the owner of this dog. The dog is being adopted as a companion and will live as a family member with me inside my home. I will provide this dog with necessary and humane care including food, fresh water, shelter, exercise, training, and veterinary care by the veterinarian of my choice. I will also provide the dog with regular professional grooming. I will not leave the dog continually tied nor allow it to run loose. I will not be cruel, mistreat or neglect this dog in violation of A.R.S. 13-2910. I will not allow the pet to be used for purposes of medical experimentation.

I have determined this dog to be an appropriate addition to my household, compatible with other pets I may have, and with all members of my family. The act of adopting the above identified dog from Arizona Poodle Rescue constitutes my understanding there are no implied guarantees or warranties concerning the temperament, health, habits, breed, age, or background of the identified dog. Any animal has the potential to bite, cause injury, or be destructive. Any costs resulting from the actions of my new dog are solely my responsibility and not that of Arizona Poodle Rescue.

I understand that this is a rescue dog. This dog may need continued training in areas such as socialization, house breaking and leash walking etc. The adopter agrees to put in the time and energy necessary to make this dog a responsible family member.

Arizona Poodle Rescue dogs come spay or neutered (unless unable due to age or medical issues) and are current on all vaccinations. NOT ALL DOGS ARE VET CHECKED PRIOR TO ADOPTION unless there are signs of illness. This decision will be made by Arizona Poodle Rescue. Adopters agree to have the dog vet checked by the Adopter’s personal veterinarian within a 14-day period. Should medical issues arise the dog may be returned to APR and the adoption fee will be reimbursed. APR WILL NOT PAY FOR ANY VET FEES AFTER THIS ADOPTION CONTRACT IS EXECUTED unless otherwise agreed upon in writing.

If the dog I am adopting is unaltered due to age or medical condition I agree to make him/her available for this procedure as scheduled by an APR representative. I understand that the procedure is to be performed by APR’s vet and at APR’s cost. If I choose to have the procedure done by my own Vet, I understand that APR is not financially responsible. I further agree to provide APR proof of sterilization if I have chosen to use my own vet.

I will always keep a collar and identification tag on the dog and will license the dog with the county/city.

I agree to immediately notify Arizona Poodle Rescue if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the dog, and to immediately retrieve the dog from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.

Arizona Poodle Rescue will provide me with the following: Adoption Contract and Health Records.

If I can no longer care for this dog, I promise to return the dog to Arizona Poodle Rescue. I promise that I will not turn the dog over to any shelter, surrender him/her to any other rescue group or sell or give him/her to another party.

Waiver and Release of Liability and Hold Harmless Agreement: In consideration of receiving this dog, I hereby release, waive, discharge, and covenant not to sue Arizona Poodle Rescue, as well as any and all of its officers, volunteers, representatives and other agents, from any and all liability, claims, costs, expenses and actions for loss, bodily injury (including death) or property damage which this animal may cause or that may otherwise arise out of or relate to the adoption of this animal. By my signature below, I certify that the information provided on this form is true, accurate and complete. I also attest that I have read and understand the foregoing Hold Harmless Agreement, and sign it freely and voluntarily, without any oral representations, statements, assurances or inducements.
